CONSTITUTING THE COMMITTEE TO WHICH ALL REPORTS OF INJURIES AND ILLNESSES OF PERSONNEL OF THIS OFFICE SHALL BE SUBMITTED FOR EVALUATION PURSUANT TO MEMORANDUM CIRCULAR NO. 210 OF THIS OFFICE, SERIES of 1968

The following are hereby designated to constitute the committee to which all reports of injuries and illnesses of personnel of this Office, whether compensable or not, shall be submitted for evaluation pursuant to the provisions of Memorandum Circular No. 210 of this Office, series of 1968:

The Chief, Administrative Office or his authorized representative Chairman

The Chief, Legal Office or his authorized representative Member

The Chief, Malacañang Clinic or his authorized representative Member

This Order shall take effect immediately.1aшphi1
