ACCOMPLISHMENT OF REVISED INFORMATION SHEET BY ALL OFFICIALS> AND EMPLOYEES OF THE OFFICE OF THE PRESIDENT

In order to provide a more comprehensive and up-to-date reference on personnel information and to facilitate the work in transacting personnel matters, all officials and employees in the Office of the President, including those who are on detail with this Office, are hereby required to accomplish in duplicate the revised information sheet and submit the same not later than March 31, 1966.

She data given in the information sheet should be complete and accurate as they will be reflected in the revised personnel record card system to be installed shortly in this Office.

Any future changes in data and information already submitted, such as address, additional educational qualifications, civil service eligibility, and the like, should be immediately reported to the Administrative Division.

Chiefs of Divisions or heads of other units shall see to it that the revised forms  accomplished by every one in their respective divisions and units. The Administrative Division may be consulted in ease additional information or assistance is desired in connection therewith. Blank forms will be distributed by the said Division and to which all the accomplished forms shall be submitted for compilation.1aшphi1
