REQUIRING THE SUBMISSION OF ADDITIONAL INFORMATION ON GOVERNMENT OFFICIALS AND EMPLOYEES WHO ARE RECOMMENDED FOR OFFICIAL TRAVEL ABROAD.

In order to facilitate the processing of requests for travel authority of government officials and employees who are recommended for official travel abroad, all heads of departments, bureaus, offices, and agencies of the government, including government-owned or controlled corporations and local governments, are hereby required to see to it that the following additional information on the personnel concerned are submitted to this Office together with the basic request:

1. Name (including middle name)

2. Place and date of birth

3. Civil status

4. Address (residence)

Strict compliance herewith is hereby enjoined.
